INCIDENT is where an event has occurred which has resulted in injury or illness. This includes psychosocial harm.
NEAR MISS is where an event has occurred but did not result in injury or illness, however had the potential to do so.
HAZARD is where no event has occurred. A situation or condition exists with the potential to cause injury or illness.
SPORTING INCIDENTS where an incident or injury has occurred during a game/competition organised by UWA Sports.

Faults, defects, or maintenance issues should be reported to Campus Management unless they have caused or contributed to a safety event. To report a fault or maintenance issue in a building or on the grounds contact Campus Management directly via this online maintenance request form. If yourfault / works require URGENT attention such as a burst pipe, power outage, toilet overflowing or lift break down, call the Campus Management Client Services Help Desk immediately on 6488 2025 (8am – 4pm Mon – Fri) or Security 6488 2222 outside of the noted hours.
